• New York State Law requires that upon entrance to Pre-K all children must have an up-to-date physical examination and proof of complete immunization, signed by a physician.
• All health records must be submitted to the school nurse before school starts.
• A nurse is available. Please be sure that you have listed an emergency phone number on your child's registration form.
• Please inform your child's teacher of any serious health condition or allergy he or she might have. All communicable diseases must be reported to the nurse.
• If your child becomes ill at school, you will be notified and asked to pick him/her up.
