Pre-K Health

New York State Law requires that upon entrance to Pre-K all children must have an up-to-date physical examination and proof of complete immunization, signed by a physician.
All health records must be submitted to the school nurse before school starts.
A nurse is available. Please be sure that you have listed an emergency phone number on your child's registration form.
Please inform your child's teacher of any serious health condition or allergy he or she might have. All communicable diseases must be reported to the nurse.
If your child becomes ill at school, you will be notified and asked to pick him/her up.
